Please complete the new patient paperwork packet prior to your visit with us. You can use a PDF editor and email the completed forms to admin@vitaliahealth.com.
Alternatively, you can print them at home and bring them completed to your appointment, or you can complete them in-office prior; please note you should arrive 15-20 minutes early in this case. Don’t forget to bring your insurance card to your appointment as well!
