PATIENTS WITH INSURANCE
Your insurance is an agreement between you and your insurance company, not between your insurance company and our office. We cannot be certain if your insurance covers Naturopathic care, although most policies do provide coverage. The amount paid varies from one policy to another.

Before your first visit, please make sure to call your insurance company and complete the Plan/Coverage Details section of the Patient Info form prior to your visit. As a courtesy to you, our office will complete any necessary insurance forms at no additional charge. It is to be understood and agreed that any services rendered are charged to you directly and you are personally responsible for payment of any non-covered services, deductibles, co-pays or co-insurance.

PATIENTS WITHOUT INSURANCE
For patients without insurance, payment for your visit is expected at the time of service. We offer a 20% time-of-service discount on services that would normally be billable to insurance. This discount is only available on the same day as your visit. We are happy to accept cash, your check, Master Card, Visa, American Express, and Discover.

FLEX PLANS/HEALTH SAVINGS ACCOUNTS
Please inform us if you have a health savings account, sometimes known as a “Flex Plan”. We will be happy to provide you with a statement of your charges so you can be reimbursed.

PERSONAL INJURY OR AUTOMOBILE ACCIDENTS
Our office can only accept a certain number of personal injury cases at any give time. All additional cases are considered on a case-by-case basis and require a brief phone consult with the doctor before scheduling. As of October 2017, we are not accepting personal injury or motor vehicle accident cases.

At the time of your appointment, please present your auto insurance card, your health insurance card, accident claim number, and tell us if you have retained an attorney. Here are options available for payment:

  1. Pay out-of-pocket for your care and we will submit reports and receipts whenever necessary.
  2. We will bill (accept assignment) from the Med Pay portion of your auto insurance.
  3. If you have retained an attorney, or if the third party auto insurance company states they are accepting financial responsibility for your accident, payment is expected at the time of service, and we will submit reports and receipts whenever necessary.
  4. We will bill your standard health insurance as long as you notify them prior to receiving treatment for your accident. You will be responsible for all co-pays, co-insurance and deductibles as they are incurred.

Although you are ultimately responsible for your bill, we will await settlement of your claim for up to 3 months after your care is completed. Once the claim is settled or if you suspend or terminate care, any fees for services are due immediately.

MEDICARE
We do not accept assignment from Medicare. Naturopaths are not considered covered providers under Medicare’s guidelines. You will be responsible for 100% of charges incurred at the time of service.

INJECTION THERAPIES
If your office visit includes injection therapies, they must be paid out-of-pocket at the time of service. Injection therapies (prolotherapy, prolozone, Platelet Rich Plasma therapy) are not covered by insurance.

SECONDARY INSURANCE
Please inform us of any secondary insurance you may have. Our office is not responsible for submitting claims to your secondary insurance provider.
